The Commonwealth Home and Community Care (HACC) Program has been developed to provide care and support services to the older community, people aged 65 years and over, and their carers who are at risk of remaining independently living, or being prematurely admitted into long term residential care.

If your organisation provides services to this demographic and would like the opportunity to enhance, expand or deliver new services under the Commonwealth HACC Program please apply before 2 September 2013 via The Department of Health and Ageing.
